Manager Clinical Quality and Population Health

Operationalizes strategies and plans for population health management across Infinity Practice sites with the goal of achieving the Triple Aim (patient outcomes, low cost and high patient satisfaction). Functions as the nurse leader at Infinity Primary Care practice sites to monitor, improve and integrate clinical practices that support safe, effective, equitable, patient-centered care. Responsible for planning, priority setting, management, coordination, implementation and evaluation of programs related to care management/care coordination. Delivers diabetes self-management education (DSME) to Infinity Primary Care patients..


  • Registered Nurse with Bachelor Degree.
  • A level of knowledge normally acquired through completion of a Bachelor’s Degree of Nursing from an accredited School of Nursing. Certified as Diabetes Educator (CDE). Certification in Care Management preferred.
  • Knowledge of Patient Centered Medical Home principles.

Associate is responsible to participate in education and training that is specific to the needs of the population(s) served by Infinity Primary Care. Associate is responsible to actively participate as a member of the physician care team. Medical Assistant (Certified or Registered)

The Medical Assistant functions as a member of the care team who actively participates in performance improvement initiatives to ensure quality patient outcomes. Assists the provider and care team with patient care and performs activities within the scope of care as defined by the American Association of Medical Assistants and within the scope of policies and procedures developed by Infinity Primary Care. 

The Medical Assistant must have a High school diploma and credentialing through AAMA (Certified Medical Assistant {CMA}) or AMT (Registered Medical Assistant {RMA}) required. If not currently credentialed, must obtain within six months of hire. Maintenance of credentials and continuing education credits necessary for certification are the responsibility of the employee. Practice Manager

The practice manager is responsible for the administrative operations of the practice and functions as a member of the care team who actively participates in performance improvement initiatives to ensure quality patient outcomes. In conjunction with the physician(s), directs the care team and assists in maintaining quality and consistency within the practice. Responsible for day-to-day operations. Examines work flow and related operational procedures and makes recommendations on improving patient outcomes, possible work simplification and cost reduction improvements.

Duties and Responsibilities:

Builds an environment that supports positive team morale. Demonstrates stellar customer service skills towards patients, medical staff, coworkers and the public. Treats staff, physicians, visitors and patients with dignity and respect. Helps create a positive and cooperative culture.

Actively supports and participates in quality improvement initiatives.Collaborates with administration and care team to identify opportunities for enhancing clinical quality, efficiency and patient experience. 

Takes responsibility for all functions of the practice and resolves issues in order to assure smooth operations. Maintains oversight of staff adherence to policies and procedures and  with pertinent regulatory and certifying entities.

Integral team member in developing capital, operations and manpower budgets while containing costs through improvements and innovations.

Actively involved in identifying opportunities for continuous quality improvement. Ability to interact with people from diverse ethnic, cultural, gender, socioeconomic and sexual orientation backgrounds. Demonstrates outstanding interpersonal communications skills. Must have a knowledge of Patient Centered Medical Home principles. 

Minimum Education:
Bachelor’s Degree preferred or comparable work experience

Work Experience:
A minimum of two years’ experience in managing a physician practice; preferably in a primary care setting. Prior experience in a Patient Centered Medical Home desired.
