SMOKING CESSATION PROGRAM
Infinity Primary Care, P.L.L.C.
“Freedom from Smoking”
Introduction: Infinity Primary Care P.L.L.C.’s smoking cessation class is an 8-week, 9-session, group cessation class. It is based on American Lung Associations “Freedom from Smoking” components, as well as behavior modification techniques, coping skills, social support, and stress management. In addition to a trained facilitator providing behavioral support, trained personnel through respiratory services and pharmacy will provide additional patient education.

Assessment
|
Assess readiness to quit, motivation, self-confidence, supportive social network, nicotine dependence level, stress level, and co-morbidities.

Program Facilitators
|
ALA trained facilitator, respiratory therapy, pharmacy.
|
|
Program Intensity
|
Deliver 9 sessions (2 sessions on week 3) over 8 weeks, each 1-1 ½ hours.
|
|
Program Format
|
Based on the American Lung Association “Freedom from Smoking” and “Nicotine Anonymous” programs. Goal is to provide patient education with an open, supportive setting.
|
|
Type of Counseling and Behavioral Therapies
|
● Use practical counseling (problem solving/skills training), such as basic information about smoking and successful quitting, developing coping skills, recognizing dangerous situations.
● Employ intra-treatment support, such as the quitting process, communicating caring and concern, encouraging self-confidence.
● Utilize extra-treatment support, such as using social support skills, arranging for outside support, and seeking social support.

Pharmacotherapy/Complimentary Therapy
|
Encourage every individual to utilize the treatment mechanism(s) that works the best for them.
|

Population
|
Support all individuals who smoke and are willing to participate in a group intervention program.
